Dear Experts,
about 6 months ago I was diagnosed with azoospermia. Overall it was a terrible experience, I ran into the problem that urologists in our area (Heidelberg), also university clinic don't know how to handle this disease (their solution is ICSI, but I disagree).
Just briefly the results:
-several spermiograms showed no sperm; I have no clue if they ever analyzed the centrifuged sample. The other values look good, pH 8, volume 3.5 mL etc.
-excellent hormone levels
-ultrasonography showed no deviations: prostate volume in the normal range, testis ~19 mL.
-even karyotype analysis was carried out (to my wife and me) - no deviations (this was done because the doctors were planning already ICSI)
- I am not aware of any infections or injuries which I could have had in my childhood.
Now come the strange results: first bacteriology analysis showed presence of Mycoplasma Hominis. I followed an antibiotics treatment and repeated bacteriology showed it was gone, but other bacteria were found: E. Coli and K. Oxytoca. Again antibiotics, repeated analysis - same E. Coli together with another bacteria. Another lab showed presence of another 2 bacteria, not the same as previous lab. Do these results have anything to do with azoospermia?
I am very confused! I believe you are the only ones who could handle this properly.
Could you please suggest what I do now?
Maybe what other analysis I should do and which ones I should repeat, just to make a more clear picture (maybe measure more parameters in the spermiogram)? If you agree, I could send you the results I already have per email.
Because Heidelberg is not that close to München, I would like to do the analysis I can do here, to at least have more understanding of my disease, before we make an appointment with you.

A diagnosis from a distance is difficult, but probably the bacteria are not important in the ejaculate. What is the FSH value?
Even with normal FSH value a non-obstructive azoospermia is the most likely cause.
If you wish, you can come to the Andrologie Centrum München with all previous medical findings.

Hello. Thanks a lot for your reply. Sorry, I saw it by chance here on the forum, did not get it by email. I feel so bad I found your answer only after 2 months.
But anyway, the blood analysis showed the following: FSH 4.8 IU/L (norm 1.5-12); LH 3.4 IU/L (norm 1.7-8.6); SHBG 39 nmol/L (norm 16-55); testosterone 5.2 µg/L (norm 2.5-8.4).
What makes you think it is non-obstructive? Do you believe there are chances for cure?
